This is our 79th year, and though there have been many changes and innovations over the years, the heart and soul of the Association remains the same—it is the people in our membership and the staff who devote themselves to improving lives by delivering quality care and services to Minnesota’s vulnerable and older adults.

Provider Membership

Any skilled nursing/nursing facility, assisted living facility, home care agency, or hospice provider licensed through the Minnesota Department of Health (MDH) is eligible to join. One membership, two associations—As a provider member of Care Providers of Minnesota, your organization is also a member of the nation's largest association of long-term and post-acute care providers, the American Health Care Association (AHCA) and the National Center for Assisted Living (NCAL), giving you access to all the resources and knowledge from your national association in Washington, DC.

Business Partner Membership

Becoming a Business Partner member gives you the strategic advantage to place your company in front of nearly 1,000 provider members across the stage. You'll have a unique connection to Care Providers of Minnesota that includes the opportunity to participate on committees and task forces, helping to shape programs and define strategies. Business Partner members also receive our weekly members-only electronic newsletter, have access to valuable information in the members-only section of our website, receive discounted and free advertising and marketing opportunities, and more.

Membership FAQ

CPM is a non-profit trade association and membership-based organization that represents and supports providers of long-term care services and support services. These include assisted living facilities, assisted living facilities with dementia care, skilled nursing facilities, and hospice agencies. CPM advocates on behalf of its members, offers education and training, and helps providers stay compliant with evolving laws and regulations.

Membership with Care Providers of Minnesota is held by organizations, not individuals. An authorized representative from your organization must submit the membership request, but all employees of member organizations can receive full access to benefits. Only an authorized representative of an eligible organization may request membership. Please check with your company representative to confirm its membership status. Membership is open to the following organization types:
  • Assisted living facilities (ALFs)
  • Assisted living facilities with dementia care (ALFDC)
  • Skilled nursing facilities (SNFs)
  • Nursing facilities (NFs)
  • Hospice providers
  • Business partners (vendors, consultants, legal firms, etc.)

Fees vary depending on facility type, size, and revenue (e.g., number of beds/units); and whether you’re a long-term care provider or vendor (business partner membership).

Most memberships run on an annual basis (January-December), renewing each calendar year.

Please check with your company to confirm whether your organization is already a member or to identify who handles membership requests. Only an authorized company representative can submit or renew membership with Care Providers of Minnesota. TIP: When filling out the initial request, make sure to include the facility/organization name and address, and Health Facility Identification (HFID).
