Federal nursing home regulations require nursing homes to prominently display written information about how to apply for and use Medicaid (Medical Assistance) and Medicare benefits, and how to receive refunds for previous payments covered by such benefits. This poster will comply with the regulatory requirements. Updated in 2016, the poster includes information about new ways to apply for Medical Assistance (Senior LinkAge Line®, MNsure Navigator, etc.) and all types of Medicare - A, B, C (including Medicare Advantage Plans), and D. The form also provides a space for the facility to indicate who within the facility questions regarding refunds should be directed to (a required component of the regulation).
