New ratings uploaded to Assisted Living Report Card

New ratings and a new maltreatment reports table were uploaded to the
Assisted Living Report Card by the Minnesota Department of Human Services (DHS).
According to the
May 18, 2026, DHS announcement, new resident health, safety and staffing ratings from surveys conducted by the Minnesota Department of Health (MDH) between Sept. 1 and Nov. 30, 2025.
In addition, DHS added a new maltreatment reports table to inform report card users of any substantiated maltreatment findings at a facility within the past 12 months.
- Users can navigate from the table to the published maltreatment report findings
- To access the table, go to the “State Licensing and Investigations Results” section of each provider’s individual quality profile page
The next planned update of the Assisted Living Report Card will be in July 2026.
