106: Polypharmacy & dementia—Lessons learned from LTC pharmacists
Audience: All audiences
How many medications are too many? Most of us understand that the more medications someone takes, the greater the risk becomes for side effects, falls, and hospitalizations. When a patient has memory loss or can no longer advocate for themselves, the risk of harm becomes even greater, and the side effects become harder to identify. These risks are further compounded when our residents take five or more medications and see multiple specialty providers. Join Kristina and Nicole, LTC consultant pharmacists, as they discuss the role of deprescribing to improve clinical outcomes and quality of life for residents living with dementia. Learn the how, when, and where to start in the deprescribing process.
