December immediate jeopardy citations

In December, the Minnesota Department of Health issued three immediate jeopardy (IJ) citations. The IJs include one F684–Quality of Care, and two F689–Free of Accident Hazards/Supervision/Devices IJs. The survey details below provide examples of IJs to assist providers with trends and prevention. The IJ synopsis’ is from the MDH findings in the 2567s and provided for the purposes of learning and preventing unintentional outcomes. See 2567 and full content on MDH website and QCOR.
December IJs details
- F684–Quality of Care: The facility did not implement prescribed treatment orders for wound care, did not notice the physician of signs and symptoms of infection and labs results; this resulted in a delay of treatment and required amputation of both toes
- F689–Free of Accident Hazards/Supervision/Devices: The facility did not assess, care plan, and ensure the correct sling was used during a transfer with a mechanical lift; the resident fell and sustained a laceration
- F689–Free of Accident Hazards/Supervision/Devices: The facility did not maintain resident supervision and safety to prevent an elopement; the resident left the facility unsupervised through an unlocked door during the night and was found outside
